Symptoms

Both NAFLD and NASH are often asymptomatic ("silent diseases") in their early stages. Possible symptoms include fatigue and pain or discomfort in the upper right abdomen (where the liver is). Because symptoms may be mild or absent, the diseases often go undiagnosed until liver damage progresses [1][3].

Diagnosing

Diagnosing NAFLD and NASH involves blood tests to assess liver function and exclude other causes, imaging such as ultrasound, CT, or MRI to detect fat and liver injury, and liver biopsy to confirm the diagnosis and assess inflammation/fibrosis, especially for NASH [1][3][5].

Treatments

Lifestyle modifications are the cornerstone for both NAFLD and NASH. This includes weight loss through healthy eating and calorie restriction, increased physical activity (e.g., 150 minutes of moderate exercise per week), and weight loss of 3-5% can improve fat accumulation; >10% weight loss may reduce liver fibrosis in NASH [3][5]. In March 2024, the FDA approved resmetirom (Rezdiffra™) specifically for NASH patients with stage 2-3 fibrosis, alongside lifestyle changes [3]. Bariatric surgery may benefit some patients with severe obesity and NASH [3]. Ongoing research continues into other drug treatments [2].

If a person has NASH, treatment focuses on managing symptoms and preventing further damage to the liver. Steatosis can initially have no symptoms and is often referred to as "silent." Around two-thirds of adults with obesity have fatty liver. A liver biopsy is the only way to confirm a diagnosis of steatosis.

Prevention is key, and people can prevent steatosis by reaching or maintaining a moderate weight, following a balanced diet, exercising regularly, and avoiding drinking too much alcohol. The Mediterranean diet is a good choice for liver health because it minimizes the consumption of processed foods, added sugar, and saturated fat.
